This explanatory page is meant to be used as a reference tool when filling out the Telephone Service Request Form.

Equipment and Service

This should be the current setup of the set except in the case of a new installation. All changes to this information should be stated under Service Request.

Primary Numberis the number assigned to the set. To test what number a set is, pick up the handset and dial 9400. You will hear a recording identifying the line number.

Class of Serviceis the restriction you want to put on the set for dialing out.

Class A: Domestic and international calls can be placed by dialing with 9 and the number.

Class B1: Domestic and international calls can be placed by dialing access code 191 for domestic calls or 192 for international calls, followed by the telephone number.

Class B2: Outgoing calls are restricted to dialing within the 617 area code and MIT community.

Class C: Outgoing calls are restricted to the MIT community, but long distance calls can be received.

Class E: All incoming and outgoing calls are restricted to the MIT community only.

Locationrequires both building and room number (example: Building E19, Room 741 would be E19-741).

Jack # should be clearly marked on the front of the beige box with two modular plugs marked 'voice' or 'data' where your telephone is connected. (For example have E19-741-1 written on the front plate of the jack in room E19-741.)

Type of Set:

ISDN 7506 and 7507 Sets are labeled on the lower right corner of the telephone face plate.

Analog Sets are basic single line telephones similar to a telephone that you might have at home.

Analog Line Only is a single voice line usually used for fax machines or computer modems where no actual telephone set is required.

7500 UDMs are data modules used to provide the user with multiple data connections.

Customer Information

Name: the name of the person who will be using the set.

Department number, optional requisition number, department contact, primary number, and location are to be filled out by the Administrative Officer or the person who authorizes changes in telephone service for the department.

Billing Information

Charges associated with the set or changes being made to the set should be billed to the appropriate seven digit cost object.

In this section, briefly describe all the work that you want done by Telephone and Voice Mail Support Services, including changes to equipment and service.

Includes any additional features or changes you might want to make on the features your set currently has. Please see the available features listed below:

Features That Can Be Added To All Sets:

Call Pickup is a group setup, that allows group members to pick up another group member's ringing line from their primary telephone. Include the primary numbers of each individual you want in the group in you request or, if there is an existing call pickup group, include one primary number already in the group.

Speed Dialing allows you to program a list of ten or twenty frequently dialed numbers into your telephone. Once these numbers are programmed, you can speed dial with * and the appropriate 1 or 2 digit speed dial code.
Note: If you have a two digit intercom, a twenty member speed dial list can not be added. Contact your Customer Service Representative for more details.

Voice Mail is a personalized information processing system. A separate account number and class of service are required to install voice mail. Please contact your Customer Service Representative or call the Voice Mail Help line at x3-3677.

Features That Can Be Added To Analog Sets:

Call Waiting signals the user on the line that a second party is calling.

Features That Can be Added to 7506 and 7507 ISDN Sets:

Dial Intercom allows the user to call persons within their group without dialing their primary number. An intercom group can contain up to 99 MIT extensions.

Auto Intercom permits one set to have direct communication with the other within MIT by pressing the feature button designated for that purpose.

Ringing Line Preference enables the user to pick up their handset and be connected to the ringing line automatically without depressing the button.

Data Access Directory Number (DADN) are numbers designated for dialing in, from off campus via the modem pool, to MIT computers connected to your ISDN telephone.

7506 and 7507 Button Layouts

Please fill in the blank spaces on the template with the appropriate telephone extensions so that it reflects how you would like your telephone to look upon completion of the work requested. Please remember to check the box if you want the appearance of the number to ring.

For example, if you have a regular digital ISDN set (7506) with your primary extension being 3-1234, and you would like it to appear 3 times on your telephone set with all the appearances set to ring, fill in the template as follows:

If you have a 20 button (7507) set and you would like several office extensions to appear on that set in addition to your own, with only your telephone extension set to ring, fill in the 7507 template as follows:

Authorized Signature

It is mandatory that the person in charge of telephone changes (preferably the administrative officer) sign the form if it is being faxed, mailed or dropped off in E19. No orders will be accepted without proper authorization. 

For Office Use Only

This is the Customer Service Representative's confirmation to you. It will have the information you need for quick reference including:

Order Received: The date the order was received.

Due date: The date assigned for the work to be completed

CSR: The Customer Service Representative who placed the order.

Service Order Number: The reference number for the order.

Facility Order Number: The reference number assigned if a jack is needed

Data forwarded to voice mail administrator: The day voice mail is to be installed, completed by CSR.

DADN: The Data Access Directory Number assigned to the set.

The Customer Service Representative will send the last copy of this form with the above filled in. Notations and modifications will be indicated on the form.
