Your Status

You DO NOT HAVE somewhere to plug the computer in (a jack).
You DO NEED an IP address.

You DO NEED a new jack installed and activated, AND a new IP address.

Contact information should identify the person in your organization who is primarily responsible for network connectivity. They will be the main point of contact between IS&T and your department for any communication relating to this request.

You also need to send us a floor plan of the area where you want wireless coverage. When you have printed out the floor plan, mark it with the case number that you receive via email that confirms this request, and fax it to 617-253-8000.

Instructions

  1. Enter the number[s] on the jack housing; enter "unmarked" if blank, and "new" for a new jack.
  2. Enter the building and room number where you need the new jack.
  3. Select the type of machine that will be connected to this jack.
  4. Enter potential hostnames and operating system for this device.

  • Contractors begin work as early as 7AM.
  • When you submit this form you will receive an email with a case number.
